In the definition of what Spiritual Health is, we can talk about the connection with inner peace. We are not only our bodies; these are just our (as the soul) vehicles to perceive the physical plane. From this stimulus, we feel, and we reason.

If you feel physically good, your body functions properly, and all your health checks come out pristine, you can tell you have good physical health. If you feel mentally focused, coherent, and functional without any problems, you can tell you have proper mental health. Even if you feel content and can interact properly day after day, you can tell you have good emotional health.

A spiritual health definition revolves around feeling good with yourself, having a good state of mindfulness while feeling connected, and having a state of comfort and balance. You will feel aligned with the path you choose to follow.

If you especially have an individual nature of spirituality and feel connected while leaving practically without fear (and taking care of what is left), you could tell that you have proper espiritual health. Deep down, if you have it, you know it.

Ultimately, you are the one who chooses your path in spirituality. You may choose a religious organization to join a communal system of beliefs and rituals, normally centered on the divine. Spirituality is more about an inner quest focusing on individual experience, aiming for meaning, the connection with the divine/universe, and finding your purpose in this existence.
